There are two options for filling out the Parish Registration Form.  Please choose #1 or #2.

1.  Fill Out the Form Online

When we receive the form an Information Packet will be sent to you.

2.  Download Parish Forms

Once you download the form, please fill it in and do one of two things:

1. bring it to Church next Sunday and give it to one of the ushers or simply put it in the collection basket,     

    or

2. mail it to  Saint John Vianney Parish.  [the address is on the home page].

3. Once you have returned the form an Information Packet will be mailed to you.
